For this book, I had a hand puppet with Velcro parts of the face to add when the story is told (similar to the picture below). It worked out great and the kids loved to be up close and personal with the scary monster. I also had the children yell "Go away!" as I took away the parts at the end.

Art Project: We made monster masks out of paper plates, various art supplies and wooden dowels to hold the mask up to their face. They cut holes out for eyes and added jewels, construction paper, feathers, stickers and other various supplies to make a great scary mask. Here's mine:
